书论著(临床研究)伴有副神经节瘤样结构的脑膜瘤(附4例报道)童贤周，吴波，董迎春，李南云，周航波，姜少军［摘要］目的脑膜瘤的组织学类型比较多，人们对有些罕见类型的脑膜瘤通常认识不足，文中通过对伴有副神经节瘤样结构脑膜瘤临床和病理的研究，提高对该肿瘤的认识。方法分析4例发生于颅内及椎管内的伴有副神经节瘤样结构脑膜瘤的临床资料，对其组织学特征、免疫表型及超微结构进行观察研究。结果脑膜瘤组织形态上具有副神经结瘤样的结构，上皮样肿瘤细胞排列成巢状，间质血管丰富，但瘤细胞表达EMA、Vimetin，而不表达Syn、CgA。电镜下瘤细胞表面有较多不规则的胞质突起，细胞间有桥粒样结构，胞质内有丰富的中间丝，而未见神经分泌颗粒。结论伴有副神经节瘤样结构的脑膜瘤具有副神经节细胞瘤的组织学特征，而免疫表型及超微结构与经典脑膜瘤一致。由于病例数的限制，其生物学行为尚不清楚。［关键词］伴有副神经节瘤样结构脑膜瘤;副神经节细胞瘤型脑膜瘤;组织化学;免疫组织化学;超微结构［中图分类号］R730．264［文献标志码］A［文章编号］1008-8199(2011)05-0499-04作者单位:243000马鞍山，安徽省马鞍山市人民医院病理科［童贤周(医学硕士)］;210002南京，南京军区南京总医院病理科(吴波、董迎春、李南云、周航波、姜少军)通讯作者:吴波，E－mail:wubo0020@sina．comMeningiomawithaZellballengrowthpattern:Areportof4casesTONGXian-zhou1，WUBo2，DONGYing-chun2，LINan-yun2，ZHOUHang-bo2，JIANGShao-jun2(1．DepartmentofPathology，Ma'anshanPeople'sHospital，Ma'anshan243000，Anhui，China;2．DepartmentofPathology，NanjingGeneralHospitalofNanjingMilitaryCommand，PLA，Nanjing210002，Jiangsu，China)［Abstract］ObjectiveMeningiomahasawiderangeofhistopathologicalcharacteristics，somerarevariantsgivearealchal-lengetotheneuropathologist．TheaimofthisstudywastogainadeeperinsightintothetumorbyexploringtheclinicalandpathologicalfeaturesofmeningiomawithaZellballengrowthpattern．MethodsWeanalyzedtheclinicaldataof4casesofmeningiomawithaZellballengrowthpatterninthebrainorvertebralcanal，andstudiedtheirhistochemicalcharacteristics，immunophenotypesandultra-microstructures．ResultsHistologically，thistypeofmeningiomawasfoundtohaveatrabecularornestlikearchitecturewithmanybloodsinuses，resemblingthatofparaganglioma．Immunohistochemically，thetumorcellswerepositiveforEMAandVimentin，butnegativeforsynaptophysinandchromograninA．Electronmicroscopyshowedzonulaoccludensanddesmosomebetweenthecells，butnoultrastructuralevidenceofneuroendocrinedifferentiation．ConclusionMeningiomawithaZellballengrowthpatternhasthehisto-logicalcharacteristicsofparaganglioma-likemeningioma，andresemblestypicalmeningiomainimmunophenotypeandultramicrostruc-ture．However，thebiologicalbehaviorofthistypeofmeningiomaremainstobeclarifiedbyfurtherstudiesofmorecases．［Keywords］MeningiomaswithaZellballengrowthpattern;Paraganglioma-likemeningioma;Histochemistry;Immunohisto-chemistry;Ultrastructure0引言脑膜瘤是较为常见的、发生于中年人脑膜部位的中枢神经系统的肿瘤。脑膜瘤的组织学类型比较多，2007年WHO的中枢神经系统肿瘤分类中，脑膜瘤的亚型有15种之多［1］，近年来有文献报道了一种新的脑膜瘤类型，称为“伴有副神经结瘤样结构的脑膜瘤”或“副神经节细胞瘤型脑膜瘤”，目前对该种脑膜瘤是否是一种新的类型还有争议［2］。我科在外检工作中遇到4例，为了提高对该肿瘤的认识，报道如下。1资料与方法1．1临床资料例1，男性，40岁，因右下肢无力进行性加重5个月，独立行走困难2d入院。头颅CT显示左侧额叶大脑镰旁见一不规则等密度影，边界欠清，大小4．0cm×4．0cm×4．5cm，CT值31Hu;左侧额骨内板可见局限性压迹，增强后病灶明显均匀强化，CT值为108Hu。在全麻下行开颅探查术，...
