第38卷第4期Vol_38No．4南华大学学报·医学版JournalofUniversityofSouthChina(MedicalEdition)2010年7月Ju1．2010·肿瘤II缶床研究·乳腺湿疹样乳癌33例临床分析王佑权。刘国文(南华大学第二附属医院肿瘤外科，湖南衡阳421001)摘要：目的分析湿疹样乳腺癌的临床特点及诊断、治疗方法，使该病的确诊提前，减少误诊、误治。方法回顾性分析33例经手术、病理证实为湿疹样乳癌患者的临床资料。结果31例患者以乳头、乳晕区糜烂等湿疹样表现就诊，伴乳腺肿块l9例，合并其它类型乳腺癌者28例；伴同侧腋窝淋巴结转移9例。初诊到确诊时间平均28．4月。13例行单纯乳房切除术，7例行乳腺癌根治术，l3例行乳腺癌改良根治术。单纯乳房切除组5年生存率为46．2％(6／13)，根治术及改良根治术组5年生存率为85％(17／20)。结论湿疹样乳癌多有典型的临床表现，但易误诊，对乳头乳晕区有湿疹样表现者均要考虑本病的可能，常规的钼靶检查有助于发现未能扪及的病灶；单纯的湿疹样乳癌可行乳房单纯切除；合并其它类型的乳腺癌者应行根治性切除或改良根治术，并行相应的辅肋治疗。关键词：乳腺湿疹样乳癌；诊断；治疗中图分类号：R737．9文献标识码：A文章编号：1672—7444(2010)04—0499—03ClinicalAnalysison33CasesofPaget’SDiseaseoftheBreastWANGYou—quart，LIUGuo—wen(TheSecondAffiliatedHospitalofUniversityofSouthChina，Hengyang，Hunan421001。China)Abstract：ObjectiveToexploreanappropriatemanagementforPaget’Sdiseaseofthebreastthroughanalyzingtheclinicalmanifestation．diagnosisandtreatment．Methods33casesofPaget’sdiseaseofbreastfromJanuary1982toJanuary2004wereretrospectivelyanalyzed．ResultsThetypicalskinchangecharacteristicofbreastPaget’Sdiseasewereeezematoidlesionofnippleandareolacomplex(31／33)．19caseshadapalpablemassinthebreast．Thereare28ca-sesassociatedwithRillunderlyingcarcinoma(ductalcarcinomainsituorinvasiveduetalcancer)and9caseshadassociatedpositiveauxiliarynodedefinitivetreatmentconsistedofradicalmastectomyormodifiedradicalmastectomyin20patients．Modifiedradicalmastectomywasoperatedin13patientsandmastectomyin7patients．13patientsweretreatedwithsimplemastcctomy．TheaveragetimebetweenpreliminarydiagnosisandfinaldiagnosisWas28．4month．Thesurvivalrateof5yearsinpatientswithsimplemasteetomywas46．2％(6／13)patientswithradicalmastectomyormodifiedradicalmastecto-myWas85％(17／20)．ConclusionPaget’Sdiseaseofthebreastiseasilytobedelayed．Eczematouschangesofthenipple—areolacomplexandpersistingsorenessoritching。withoutobdousreasonIisasuspicioussymptomforPaget’Sdis-easeofthebreastandcallsforthoroughevaluation．Regularmammographyexaminationsaidinearlydetectionofoccultdis-ease．MastectomyistheappropriatetreatmentforPaget’Sdiseaseofthebreastwithoutunderlyingmass．ThepalpablecaseswererelatedtoasignificantlyhigherincidenceofinvasiveduetalcancerandaWorseprognosis，whilemodifiedradicalma8一tectomyfollowedbyadjuvanttherapyshouldbeperformed．Keywords：Paget’Sdiseaseofthebreast；diagnosis；therapeutics乳腺湿疹样癌(又名Paget’S病)是以乳头和乳晕部出现湿疹样改变为主要临床表现的特殊类型收稿日期：2010-05—04通讯作者：王佑权，硕士研究生，主治医师，E—mail：youqw@163．com．499乳腺癌，临床少见。该病以乳头乳晕区皮疹开始，呈颗粒状、深红色类似湿疹样病变，病程发展较慢，极易误诊为乳头乳晕区湿疹，确诊时间长。单纯的Paget’s病预后较好，合并其它类型的乳腺癌者，预后较差，误诊后严重影响患者的预后。现回顾性分析本院自1982年1月一2004年1月收治的33例Paget’s病的临床资料，以提高对本病的认识，争取做到早诊断、早治疗，以提高疗效。1资料与方法1．1一般资料Paget’s疾病患者33例，女性，均单侧发病，发病年龄38～7l岁，...
