中国组织工程研究与临床康复第13卷第50期2009–12–10出版JournalofClinicalRehabilitativeTissueEngineeringResearchDecember10,2009Vol.13,No.50P.O.Box1200,Shenyang110004cn.zglckf.com9946肌腱损伤修复过程及粘连的预防★刘汉辉1,2,章莹1,吴文1,谭新宇1,2RepairoftendoninjuryandpreventionofadhesionLiuHan-hui1,2,ZhangYing1,WuWen1,TanXin-yu1,2Abstract:Tendonhealingfollowinginjuryincludestwoforms,endogenoushealingandexogenoushealing.Theidealwayofpreventingadhesionoftendonistoimprovethenutritionalstateandthesurroundingenvironmentoftendonsoastopromoteendogenoushealingandrestrain/easeexogenoushealing.Thenormally-usedmethodforpreventingtheadhesionincludes:systemicorpartialadministration,protectionandrepairoftendinoussheath,substitutionoftendinoussheathwithautologoustissueorsyntheticmaterial,tendontransplantation,tendinoussheathreconstructionandsoon,allofwhichdohavesomecurativeeffects.However,recentresearchfocushascomedowntothedegradableandabsorbablepolymericbiomaterialsthatcanbeusedasbarriersforpreventingtheadhesion.Besides,traditionalChinesemedicine,rehabilitationtherapyandearlyprotectedmotionallcanpreventtheadhesiontosomedegree.Infuture,thetissueengineeringtendontransplantationmaybecomeaninvestigativeaspect.Byusingatraumatictechniqueandnon-strangulatedsutureintheearlierperiodaftertendoninjury,wardingandrepairingtendinoussheathasmuchaspossibleaswellaspackagingthetendonwithpolymericbiomaterialsduringoperation,andadoptingcombinedtherapyofrehabilitationtherapytogetherwithearlyprotectedmotionafteroperation,wecanpreventtendonadhesionobviouslyandhavetheglidingfunctionoftendonrecoveredtothegreatestextent.LiuHH,ZhangY,WuW,TanXY.Repairoftendoninjuryandpreventionofadhesion.ZhongguoZuzhiGongchengYanjiuyuLinchuangKangfu.2009;13(50):9946-9949.[http://www.crter.cnhttp://en.zglckf.com]摘要:肌腱损伤后愈合包括内源性愈合、外源性愈合两种形式。防止肌腱粘连的理想途径,是通过改善肌腱的营养状况和周围环境,来促进内源性愈合,抑制或减轻外源性愈合。常用的预防肌腱粘连方法有:全身或局部应用药物、保护和修复腱鞘、采用自体组织或合成材料代腱鞘、肌腱移植与腱鞘再造等,均取得了一定效果。采用可降解吸收的高分子生物材料屏障预防肌腱粘连,是近年来临床应用的热点。采用中医中药疗法、康复理疗及早期保护性活动,均可一定程度预防粘连发生。组织工程化肌腱移植,可能是今后研究的一个方向。在肌腱损伤早期,采用无创技术、非绞窄性缝合,手术中尽量保留或修复腱鞘,外周应用高分子生物材料包裹,术后结合康复理疗及早期保护下功能锻炼的综合治疗方法,可明显预防粘连发生,最大限度地恢复肌腱的滑动功能。关键词:肌腱损伤;修复;粘连doi:10.3969/j.issn.1673-8225.2009.50.030刘汉辉,章莹,吴文,谭新宇.肌腱损伤修复过程及粘连的预防[J].中国组织工程研究与临床康复,2009,13(50):9946-9949.[http://www.crter.orghttp://cn.zglckf.com]0引言肌腱修复后长时间制动常常造成粘连,严重影响滑动功能,从而使肢体运动功能恢复不满意[1-2]。肌腱粘连常因需行二期松解术而增加患者的痛苦和经济负担。因此,如何有效预防肌腱粘连的发生,是很多学者一直努力解决的问题。1学术背景肌腱由大量平行排列的胶原纤维组成,基本单位是胶原纤维束,但束与束间相互交替,防止纤维束间的分离[3]。肌腱外周为结缔组织和滑膜脏层包绕肌腱表面形成的腱外膜,其部分纤维向肌腱内延伸,在肌腱束间包绕构成腱内膜,肌腱系膜由来自滑膜鞘的壁层滑膜,以及披覆肌腱末端的脏层(腱外膜)相连续形成。腱系膜成半透明状,其中有血管、淋巴管和神经,供给肌腱营养。肌腱的腱周组织、滑膜鞘、纤维鞘管及肌腱的支持带等组织是保障肌腱滑动、营养、发挥肌腱功能的重要结...
