1腔镜胰十二指肠切除术与开腹胰十二指肠切除术治疗胰腺癌的系统评价及荟萃分析中 文 摘 要目 的 ： 本 研 究 的 目 的 是 比 较 腹 腔 镜 胰 十 二 指 肠 切 除 术 (Laparoscopic pancreatoduodenectomy ， LPD) 和 开 放 胰 十 二 指 肠 切 除 术 (Open pancreaticoduodenectomy，OPD)对胰腺癌(Pancreatic cancer，PC)患者的肿瘤预后和临床疗效。方法：本荟萃分析通过计算机系统地检索了 PubMed、EMBASE、Web of Science和 Cochrane 图书馆进行搜索检索，截止日期至 2019 年 10 月，根据纳入标准及排除标准筛选出符合条件的研究，并使用新渥太华量表(NOS)来评估纳入研究的偏倚风险，通过 GRADE 评分法进行证据质量评估。通过荟萃分析比较了 LPD 和 OPD 治疗PC 的疗效，对肿瘤结果和围手术期资料进行分析。结果：此次荟萃分析包括 8 项研究共涉及 13809 名患者。两种手术方式的 5 年总生存率（OS）无显著性差异（HR:0.97，95%CI 0.82–1.15，p=0.76）。LPD 的 R0 切除率较 OPD 无明显统计学差异（OR:1.16，95%CI 0.85-1.57，p>0.05）。本研究显示，与 OPD 相比,LPD 术后胰瘘发生率(OR:1.07,95%CI:0.68-1.68,p=0.77)及术后出血(OR:1.74,95%CI ： 0.96-3.71,p=0.07) 相 似 ， 更 多 的 淋 巴 结 被 切 除（WMD:1.84，95%CI:0.95–2.72，p<0.05），住院时间缩短（WMD:-2.45，95%CI:-3.33- -1.56，p<0.05），估计失血量较少（WMD：-374.30，95% CI：-513.06- -235.54，p<0.05）。关键词：腹腔镜胰十二指肠切除术，开放胰十二指肠切除术，PC，肿瘤预后2 n oA Comparative Study of Ileal Loop-type Ileostomy and Modified Terminal Ileum Cannula Ileostomy in Anus Preservation Operation Low Rectal CancerAbstractPurpose: The purpose of this study was to compare the tumors of patients undergoing laparoscopic pancreatoduodenectomy (LPD) and open pancreaticoduodenectomy (OPD) to patients with pancreatic cancer (PC).
